NC Return to Play Written Clearance Forms

These are the ONLY forms accepted after a concussion. This is to be filled out by a LHCP (Licensed Health Care Provider - example MD/DO), for concussion management and gradual return to play. ONLY a LHCP (Licensed Health Care Provider - example MD/DO) can manage a concussion and clear an athlete to start the 6-step "Gradual Return to play plan".
*ALL atheltes (NC middle school and NC high school) MUST to go through this 6-step "Gradual Return to Play Plan" in order to particpate in athletics after a concussion, when the doctor feels the athlete is ready. MAKE SURE the LHCP (Licensed Health Care Provider - example MD/DO), completes ALL the necessary forms each time there is a change in the athlete's progression to learning - school and/or to return to play - PE and sports.
